Bring Your Medication List
Include prescription medicines, over-the-counter medicines, vitamins, and supplements. The National Institute on Aging recommends keeping an up-to-date list and discussing it with your clinician.
For Family Members
If you are helping a parent or another family member, call us about arranging the visit. Ask the office what information and authorization are needed before discussing their care.
